Reimbursement of claim:
Reimbursement facility can be claimed from insurance co. after u have been discharge from hospital.
In case of hospitalization, in non-network hospital the insured is required to intimate the TPA 3-4 days before in case of planned hospitalization and 24 hours before in case of emergency hospitalization.
After discharge the claim form should be submitted within 7 days along with following documents.
Documents to be submitted:
- Mediclaim policy with schedule and endorsement /clause.
- Claim form completely filled and signed by the claimant.
- Doctor's advice for hospitalization and his first prescription.
- Original discharge card/ summary from the hospital.
- All diagnostic test reports e.g. X-Ray, E.C.G, USG, MRI Scan,etc. supported by medical(pre & post hospitalization).
- All medicines purchased from outside the hospital.
- Surgeon's certificate stating the nature of the operation performed and surgeon's bill and receipt.
- Copy of the policy document.
